Each hospital gives each patient a unique number. This can be found on the patient's wrist band and can be known as a Medical Record Number or Universal Record Number. It is usually 6 or 7 digits long. Please ask hospital staff for clarification.

Packages are non-refundable. Balance credit can be held for 6 months if the patient leaves early or changes their mind.
